This waiver requiring digital signature will be emailed to the address you provide after registration. This waiver MUST be signed before the first day of class!

Cancellation Policy:

For group classes, please ensure you can attend all 6 weeks of class before enrolling. Fees non-refundable after registration, as you have filled a class slot that may not be able to be resold before the start of class. In lieu of cancellation, you may transfer your dog to a different class start date for a transfer fee by contacting Jenna at 217-607-9909 or at training@bestpawforwardil.com. If a transfer is requested at least 7 days prior to your original class start date, the transfer fee is $20. Transfer fee for requests less than 7 days prior to original class start date is $35. Transfer fee is waived in the event of transmissible canine illness. Classes cannot be resold to a third party, the attendee must be as per the booking name. If registration is made over the phone, an invoice will be emailed to the email you provide. Payment of is required to secure your spot in class. You are not enrolled in the class until payment is received

Vaccination Policy

Your dog must be up to date on DHLPP (or equivalent) and Bordetella per facility policy. Briggs Republic requires dogs to be vaccinated twice a year for Bordetella, so please ensure you dog has been vaccinated for Bordetella within the last 6 months. For dogs older than 4 months it is required by state law for them to be up to date on the Rabies vaccine prior to receiving services. All vaccines must be administered by a licensed veterinarian. Vaccines purchased from farm and fleet or tractor supply and administered by the owner do not count towards legal vaccination requirements.

Location Agreement (Briggs Republic Students Only)

I hereby agree to indemnify and hold harmless Briggs Republic, its owners and independent contractors of and from any and all claims, causes of action, losses, damages, judgments, costs or expenses, including court costs and attorney’s fees, which in any way arise from my or my family member’s or guest’s acts, omissions and/or negligence, irrespective of whether such acts, omissions and/or negligence of Briggs Republic, its owners and independent contractors or any third party contributed to the loss.

Training Agreement

This document records the agreement of the undersigned owner of any dog (A) being trained at or otherwise utilizing the facilities of Best Paw Forward, LLC, (B) participating in any event in which Best Paw Forward, LLC l is acting, at its facility or other premises, as a sponsor, facilitator or participant of and at a dog training or competition event, training class, in-home training visit, training session occurring at any public location, board and train, exhibition, show or fundraiser (together “Activity”), that within the scope hereof Best Paw Forward, LLC includes its owners and independent contractors while performing their
respective Best Paw Forward, LLC duties and actions, and (D) as such owner I hereby affirm:

A. I understand that my attendance and participation in any Activity is not without risk to myself, my dog(s) or any family member or guest who might be present at an Activity. Some dogs to which I or my dog might be exposed may be difficult to control, may act aggressively towards other canines or owners and may be a cause even when handled with care. In addition, while it is my responsibility to keep my dog current on vaccinations, I recognize that dogs may still be susceptible to various diseases or infections in spite of the vaccinations and may infect me or my dog. I hereby assume the risk of incurring any such infection or injury while attending any Activity and expressly release Best Paw Forward, LLC, its owners and independent contractors of and from any and all liability of any nature, for injury or infection which I, my dog or any family or guest may sustain, including specifically, and without limitation, any injury or infection caused by the action of another dog while participating in an Activity.

B. I specifically understand that I am releasing, discharging and waiving forever any claim or action that I may have had, presently have, or may hereafter have for acts or other conduct by the owners or independent contractors of Best Paw Forward, LLC.

C. In further consideration of the foregoing and as an inducement to my participation in any Activity, I hereby agree to indemnify and hold harmless Best Paw Forward, LLC, its owners and independent contractors of and from any and all claims, causes of action, losses, damages, judgments, costs or expenses, including court costs and attorney’s fees, which in any way arise from my or my family member’s or guest’s acts, omissions and/or negligence, irrespective of whether such acts, omissions and/or negligence of Best Paw Forward, LLC, its owners and independent contractors or any third party contributed to the loss.

D. I am aware and agree that Best Paw Forward, LLC or its authorized photographer may take photographs of me, my family and guests and my dog(s) for Best Paw Forward, LLC use in promotional materials including, but not limited to, printed material, website, press releases and fliers.

IN WITNESS WHEREOF, each of the parties in consideration of these presents has duly executed this Agreement as of the date set in this form.
